患者への声かけ・挨拶の基本フレーズ
患者に安心感を与えるためには、最初の声かけが重要です。穏やかで明確な言葉を使い、患者が理解しやすいスピードで話すことを心がけましょう。
病室訪問・初対面のフレーズ
Good morning, Mr. / Ms. [name]. My name is [name], and I'll be your nurse today.
おはようございます、[名前]さん。私は[名前]と申します。本日担当の看護師です。
How are you feeling today? / How did you sleep last night?
今日のご気分はいかがですか？/ 昨夜はよく眠れましたか？
I'm here to check on you and take your vital signs.

バイタルサイン確認・測定のフレーズ
バイタル測定は日常業務の中核です。患者に何をするかを事前に説明してから始めることで、不安を軽減できます。
血圧測定
I'm going to take your blood pressure now. Please extend your arm and relax.
これから血圧を測定します。腕を伸ばしてリラックスしてください。
You may feel some pressure on your arm — that's normal.
腕に少し圧迫感があるかもしれませんが、正常です。
Your blood pressure is 120 over 80. That's within the normal range.
血圧は120/80です。正常範囲内です。
体温・心拍・酸素飽和度
I'm going to take your temperature. Please open your mouth and place this under your tongue.
体温を測ります。口を開けて、これを舌の下に置いてください。
I'll put this clip on your finger to check your oxygen level and heart rate. It won't hurt.
酸素濃度と心拍数を確認するため、指にクリップを付けます。痛くありません。

体重測定・その他
Can you step on the scale for me? Please remove your shoes first.

処置・ケア内容の説明フレーズ
処置を行う前に、何をするか・どのような感覚があるかを説明することで、患者の不安を和らげることができます。
点滴・採血の説明
I'm going to insert an IV line now. You'll feel a small pinch.
これから点滴のラインを入れます。少しチクッとします。
Please make a fist for me. I need to draw some blood for a test.
手を握ってください。検査のために採血します。
The IV drip should take about two hours. Please try not to bend your arm.
点滴には約2時間かかります。腕を曲げないようにしてください。
内服薬・注射の説明
It's time for your medication. Here are your pills — please take them with this water.
お薬の時間です。こちらが薬です。この水で飲んでください。
This is an injection. You'll feel a slight sting for a moment.
注射です。一瞬チクッとします。
I'm going to change your dressing now. Tell me if it's too painful.
今から包帯を交換します。痛すぎる場合は教えてください。
体位変換・移動の介助
I'm going to help you turn over. Ready? On the count of three — one, two, three.

痛み・状態のアセスメント
患者の痛みや状態を正確に把握するためのアセスメントフレーズです。スケールを使った痛みの評価は、英語でも広く使われています。
Can you describe your pain? Is it sharp, dull, or burning?
痛みを説明していただけますか？鋭い痛み、鈍い痛み、灼熱感のどれに近いですか？
On a scale from 0 to 10, with 0 being no pain and 10 being the worst pain imaginable, how would you rate your pain?
0が痛みなし、10が想像できる最大の痛みとして、0から10で痛みをどのくらいと表しますか？
Is the pain constant, or does it come and go?
痛みは持続していますか、それとも断続的ですか？
Does anything make the pain better or worse?
何か痛みが和らいだり、悪化したりすることはありますか？
Have you had any nausea, vomiting, or dizziness since last night?

退院指導・服薬指導のフレーズ
退院時の指導は、患者が安全に回復するために欠かせないステップです。わかりやすく、確認を交えながら説明しましょう。
服薬指導
You have three medications to take at home. Let me explain each one.
自宅で飲む薬が3種類あります。それぞれ説明させてください。
Take this pill twice a day — once in the morning and once at night — with meals.
この薬は食事と一緒に、朝と夜の1日2回飲んでください。
Do not skip doses, even if you feel better. Complete the full course.
気分が良くなっても飲み忘れないでください。全コース飲み切ってください。
If you experience any side effects such as rash, difficulty breathing, or severe stomach pain, call us or go to the emergency room right away.
発疹、呼吸困難、強い腹痛などの副作用が出た場合は、すぐにご連絡いただくか、救急に行ってください。
生活指導・フォローアップ
Please rest as much as possible for the next few days and avoid strenuous activity.
今後数日間はできるだけ安静にして、激しい運動は避けてください。
